Abstract

|
목적: 본 연구는 현재 응급의료기관(ED)에서 사용하고 있는 한국형 응급환자 분류도구(Korean Triage and Acuity Scale, KTAS)의 간호사-응급구조사 간 신뢰도와 KTAS 분류 등급의 불일치 영역을 평가하기 위한 전향적 조사연구이다.
방법: 인천광역시에 소재한 응급의료센터에 내원한 15세 이상의 성인과 15세 미만의 소아 환자를 대상으로 각 400명씩 총 800명을 편의 표집 후, 2019년 1월 11일부터 5월 31일까지 평가자 간의 신뢰도를 평가하였다. 중증도 평가자는 응급실 포함 임상경력 7년 이상의 간호사 2인과 응급구조사 2인이며, 환자 분류소에서 실제 환자를 대상으로 간호사 1인과 응급구조사 1인이 동시에 중증도 분류(KTAS)를 시행하였다. 중증도 분류 평가자 간 신뢰도(Inter-rater reliability)는 weighted kappa로 분석하였다.
결과: 성인 KTAS의 간호사-응급구조사 간 신뢰도는 weighted kappa 값이 .71로 ‘상당한(substantial)’ 수준이었고, 소아 KTAS의 간호사-응급구조사 간 신뢰도는 weighted kappa 값이 .66으로 ‘상당한(substantial)’ 수준으로 나타났다. 불일치 영역으로 성인의 경우 가장 빈번한 것은 ‘통증 강도의 측정 차이’였다(28.0%). 성인과 소아 환자 모두에게 ‘복합 주증상에 따른 분류 항목의 선택 차이’가 상위 불일치 요인으로 확인되었으며, 소아 환자에서는 주관적 판단에 따른 체온 평가 항목의 불일치도가 가장 높았다(50.7%).
결론: 따라서 KTAS는 합리적인 응급의료체계의 표준화된 중증도 분류 도구로서 적절한 신뢰도 수준임을 확인하였다. 또한 평가자 간 신뢰도 수준 및 분류 능력 향상을 위해 현재 통합되어있는 KTAS 제공자 코스의 직군별 차별화 된 교육과정으로의 개정이 필요하다.

Purpose: This study was a prospective survey to evaluate the Korean Triage and Acuity Scale (KTAS) inter-rater reliability and to identify discordant areas in the KTAS classification between nurses and paramedics in emergency department (ED).
Methods: Inter-rater reliability were evaluated using triage deta from January 11 to May 31, 2019. A convinience sample of a total of 800 patients who visited an emergnecy medical center in Incheon (400 adult and 400 pediatric patients in this study) were selected. The triages of this study a pair of one nurse and one paramedic performed triage at the same time, using the KTAS, Inter-rater reliability for the KTAS was evaluated with the weighted kappa.
Results: The inter-rater reliability of the KTAS evaluated by weighted Kappa between nurse-paramedic KTAS score of adult patients was .71, and .66 for pediatric patients. Both were in the level considered as substantial. Among causes of triage discordance, the most frequently identified discordant item in triaging adults was the difference in the measurement of pain intensity (28.0%). For both adult and pediatric patients, multiple chief complaint was identified as the major discordant factor.
In triaging pediatric patients the body temperature assessment by subjective judgments was the highest discordant item (50.7%).
Conclusion: The study result suggested that the KTAS demonstrated a reasonable level of inter-rater reliability and functioned as a standardized triage tool for emergency medical services. In order to improve the inter-rater reliability and classification competence, it is necessary to revise the current all-integrated KTAS provider course to be differentiated for each job group.
